Job details:  Transfer of Care Hub. General Duties: Acting as a source of therapy expertise within the TOCH.
Working to resolve any therapy issues that may be delaying discharge, including the ordering of equipment where required
Working closely with acute and community therapy colleagues to implement a pull discharge model with an emphasis on Home First
Acting as an expert assessor to optimise discharge pathways where the TOCH request that Home First is re-explored
Working closely with therapy leads to identify and address therapy issues that result in additional delay days
Supporting transformation schemes to improve discharge performance and optimise the TOCH
Working hours: Monday to Friday, 08.00/08.30 – 16.00/16.30.
