Job details: SDH&C Home First Service. To start asap until permanent recruitment. Car driver with access to a car essential. Experience of working in MDT for UCR and discharge support in community, home visits with team/alone for assessment, therapy provision, equipment provision etc. Working alongside RSW. Working hours: Long days (8am-8pm with 40 min break)
