Tripod Partners are currently recruiting for a Community Psychiatric Nurse to work with the MH Team in Swindon
This will include the assessment, monitoring, and provision of specialist community psychiatric nursing interventions to individuals with mental health needs within the community. This will also involve ongoing risk assessment and the development of comprehensive care plans. You are responsible for coordinating care, including referrals and ensuring that aftercare is arranged in accordance with the Care Programme Approach (CPA). Care should be integrated and coordinated with other services, such as social services, voluntary organisations, and primary care, as required.
Provide a range of interventions across the care spectrum; this will include delivering primary and secondary mental health care interventions based on evidence-based practices, supporting individuals with severe and enduring mental health problems. Collaborate closely with multidisciplinary teams, including social workers, psychiatrists, psychologists, and substance misuse services, considering cultural, racial, and religious considerations in the delivery of care.
Act upon referrals from General Practitioners (GPs), other CMHT professionals, social services, voluntary organisations, and acute services. You will provide thorough assessments and develop tailored care plans, ensuring that the needs of service users on your caseload are met, with appropriate mental health interventions being implemented, monitored, and reviewed regularly.
Maintain awareness of safeguarding policies, including those related to vulnerable adults, children, and the Mental Capacity Act (2005). Work within this framework to ensure that the rights of individuals are upheld, and their safety is prioritised.
Work within the Mental Health Act 1983, applying robust knowledge of its provisions to community settings, including sections relating to compulsory admission and treatment. You will collaborate with other professionals to ensure the Act is applied correctly and ensure that individuals are provided with the care and support they need in line with legal frameworks.

To carry out timely assessments on children, young people and families in accordance with the Framework for Assessment and working together guidance. 3. Ensure assessments and planning for children and young people actively promote the participation and engagement of all relevant agencies, the child/young person and their family. 4. Prepare/formulate care plans for children and young people which take account of their views wishes in the implementation of those care plans. 5. To maintain case records to a high standard and produce reports in accordance with the Service guidance/policy and procedure which reflect national guidelines using the relevant information technology. 6. Assess the need for social work service including the identification of risk and the need for protection, determine the method of intervention, and decide and advise on the use of appropriate social services and/or other resources. 2. Liaise and negotiate with other professionals, statutory and voluntary agencies to ensure that the best possible service is provided for clients. 3. Manage and monitor an allocated workload within individual and group priorities and Council policies, and comply with the statutory obligations of the Group. 4. Maintain appropriate records of work undertaken and carry out required administrative procedures. 5. Prepare for and attend supervision sessions and staff meetings and make use of all available training and development opportunities. 6. Contribute to the evaluation and development of services and new ideas by sharing knowledge about theory, skills and practice with other social services staff, professional groups and interested bodies. 7. Supervise students where appropriate.

In addition to undertaking complex assessments and family finding. • To produce reports, attend planning meetings, panel, court, reviews, team meetings, Psychology Service meetings and inter-agency meetings as necessary or appropriate. • To mentor and support less experienced social workers. • To contribute to service and policy development and lead on identified prioritise
