Tripod Partners are currently recruiting for a Community Psychiatric Nurse to work with the MH Team in Hertfordshire

Key Responsibilities


 
  • This will include the assessment, monitoring, and provision of specialist community psychiatric nursing interventions to individuals with mental health needs within the community. This will also involve ongoing risk assessment and the development of comprehensive care plans. You are responsible for coordinating care, including referrals and ensuring that aftercare is arranged in accordance with the Care Programme Approach (CPA). Care should be integrated and coordinated with other services, such as social services, voluntary organisations, and primary care, as required.
  • Provide a range of interventions across the care spectrum; this will include delivering primary and secondary mental health care interventions based on evidence-based practices, supporting individuals with severe and enduring mental health problems. Collaborate closely with multidisciplinary teams, including social workers, psychiatrists, psychologists, and substance misuse services, considering cultural, racial, and religious considerations in the delivery of care.
  • Act upon referrals from General Practitioners (GPs), other CMHT professionals, social services, voluntary organisations, and acute services. You will provide thorough assessments and develop tailored care plans, ensuring that the needs of service users on your caseload are met, with appropriate mental health interventions being implemented, monitored, and reviewed regularly.
  • Maintain awareness of safeguarding policies, including those related to vulnerable adults, children, and the Mental Capacity Act (2005). Work within this framework to ensure that the rights of individuals are upheld, and their safety is prioritised.
  • Work within the Mental Health Act 1983, applying robust knowledge of its provisions to community settings, including sections relating to compulsory admission and treatment. You will collaborate with other professionals to ensure the Act is applied correctly and ensure that individuals are provided with the care and support they need in line with legal frameworks.

Position Details:
Work Pattern:37.5 hours per week, 09:00-17:00, Monday – Friday.

Duration: Initially 3 months likely to be extended further 
Pay Type: Umbrella or PAYE available

Job details:  3x Paediatric Occupational Therapists required for a weekend service across Watford, Stevenage, and Hertford to start ASAP. Skills/experience: must have at least 1 years NHS experience. Qualified Occupational Therapist with current HCPC registration. Children’s OT experience in the last two years (essential). Experience of undertaking paediatric Occupational Therapy assessments (essential) including the Movement Assessment battery for Children, Developmental Test of Visual Motor Integration, Detailed Assessment of Speed of Handwriting. Experience implementing a range of interventions (essential). Undertaken relevant CPD within the last two years (essential). Experience of working with a range of occupational participation difficulties e.g.  self-care and school based occupational performance difficulties (essential). Ability to manage own caseload effectively (essential). Experience of working as an OT in a community clinic or mainstream school setting within the last two years (essential). Max 12 clinics each weekend.
Duties:
OT to support with Health and EHCP. For health age range is 5-18.
Health OT: Functional difficulties. Personal activities. Access to home.
OT will complete initial assessment for the child.  Complete Child scoring, targets and report.  Complete any child referral.
If Child needs interventions will be referred to HCT own therapy assistant.  Some children may have interventions with OT, but most will be in by assistant. Max of 6 interventions per child.
Initial assessment 90 minutes, plus admin, plus referrals, plus reports.  Max 2 Children per day.
Report to then be sent to parent, school, GP and original referrer. 
If OT does intervention 45 – 60 minutes, plus admin
Working hours: Weekends only. Must be able to commit to a minimum of 2 weekends per month. 9-hour days, 08.30 – 07.00 (1 hour lunch unpaid each day, will consider shorter lunch with earlier end time)
