Band 6 CPN Care Coordinator – Early Intervention Team

Tripod Partners are currently seeking a skilled Band 6 Community Psychiatric Nurse (CPN) to join the Essex Support & Treatment in Early Psychosis (ESTEP) team. This role is based at Laurel House, Rochford Hospital, and focuses on providing care coordination for early intervention in psychosis.

Key Responsibilities:

• Act as a Care Coordinator for individuals within the Early Intervention pathway.
• Deliver comprehensive mental health assessments and care planning.
• Provide therapeutic interventions tailored to patient needs.
• Work collaboratively with a multi-disciplinary team to ensure seamless patient care.

Position Details:

• Location: Rochford Hospital, Rochford, Essex SS4 1RB
• Work Pattern: Monday to Friday, 9:00 AM – 5:00 PM
• Start Date: 3rd February 2025
• End Date: 3rd April 2025
• Rate of Pay: £26.87 per hour
• Pay Type: Umbrella or PAYE available

Requirements:

• Registered CPN with experience in mental health care coordination.
• Previous experience working within Early Intervention or similar services.
• Strong skills in mental health assessment, care planning, and delivering interventions.
