Havering Council is looking for a committed and skilled Social Worker to join our Adults’ Learning Disabilities Team. This role is essential in supporting adults with learning disabilities to lead independent, safe, and fulfilling lives. You will work within a strengths-based, person-centred framework to assess needs, coordinate care and support plans, and safeguard the rights and well-being of vulnerable residents in line with the Care Act 2014.
Key Responsibilities:

Undertake Care Act assessments and reviews for individuals with learning disabilities, ensuring person-centred outcomes and co-produced support plans.

Work closely with health professionals, families, advocates, and providers to deliver integrated, high-quality care and support.

Carry out Mental Capacity Act assessments and contribute to safeguarding adult enquiries under Section 42 when required.

Maintain up-to-date and accurate records in accordance with Havering Council’s policies and national legislative requirements.
