Occupational Therapist
Brief Job Overview
Tripod Partners are recruiting an Occupational Therapist for a locum position within Social Services. This role supports both the hospital team and the reablement service, helping individuals regain independence and safely transition from hospital to home.
Working as part of a multidisciplinary environment, the Occupational Therapist will assess individuals’ functional abilities following hospital admission and contribute to discharge planning and reablement support. The role includes working within a hospital setting for part of the week and collaborating with community-based teams to ensure individuals receive the appropriate support to live safely and independently.
Key Responsibilities
- Carry out occupational therapy assessments for individuals within hospital and reablement services.
- Support discharge planning by identifying needs related to daily living and independence.
- Work collaboratively with health and social care professionals to coordinate safe and timely discharge from hospital.
- Develop person-centred intervention plans that support individuals to regain or maintain independence.
- Recommend appropriate equipment, adaptations, or reablement interventions where required.
- Maintain accurate records and documentation in line with professional and organisational standards.
- Contribute to multidisciplinary discussions regarding care planning and rehabilitation outcomes.
About You
You will be a qualified Occupational Therapist with experience working within hospital discharge, community rehabilitation, or adult social care services. You will be confident assessing functional needs, identifying appropriate interventions, and supporting individuals to regain independence following hospital admission.
Strong communication skills and the ability to work collaboratively with multidisciplinary teams are important in this role, particularly when coordinating discharge planning and reablement services.
Requirements
- Recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Experience working within hospital discharge, reablement, or adult social care services.
- Ability to assess functional needs and recommend appropriate equipment or interventions.
- Strong communication and multidisciplinary working skills.
