Job details:  SDH&C Home First Service.  Attending ward/board rounds, liaising with acute and community services, follow up D2A visits in the community. Skills/experience: Community experience, inpatient experience, D2A experience. Car driver essential. Working hours: 37.5 hours per week over 4 days – initially Monday to Friday but potentially will extend into the weekend.
