Job details:  Transfer of Care Hub. Skills/experience: Complex discharge planning and acute NHS hospital experience.
Working hours: 37.5 hours per week. Monday to Friday 08.00 – 16.00. Also includes rostered evening shifts up to 8pm and rostered weekends.
