Job details:  Adults & Elder. 1.0 WTE. Speech & Language Therapist required for 3 months to tend to Increasing waiting lists, increasing urgent swallowing referrals, with palliative care needs. To provide specialist speech and language therapy to adults with acquired communication and / or swallowing difficulties.  The post holder will work across two services, the Adult Speech and Language Therapy Service and the Community Neuro Service.  The Adult Speech and Language Therapy Service provides assessment, treatment and management to a wide range of adult clients with acquired communication and / or swallowing difficulties in a community setting.  The caseload is varied, and includes adults over 16 years of age, with acquired speech, language and swallowing difficulties secondary to, for example, dementia, other mental health conditions, respiratory conditions, gastro conditions, cancer, palliative and end of life care.  Clients are seen within their own homes and other appropriate domiciliary settings.  The service also provides input to three adult inpatient wards, including older adult mental health, and step-down ward.  The Community Neuro Service (CNS) is a multidisciplinary team that provides rehabilitation and management for people who have had a stroke or have a neurological condition in their own home or appropriate community location.  The service provides specialist, coordinated advice and treatment to maximise independence, limit disease progression, reduce hospital admissions and facilitate smooth transition home following a hospital discharge. Working hours: 37.5 hours per week.
