Job details:  Acute adult inpatients. Ongoing – approval one month at a time. The assessment of older patients on the inpatient wards to ensure safe discharge including provision of equipment necessary for discharge and referral on to the most appropriate discharge pathway, working closely with physio colleagues. Working hours: Monday to Friday, 09.00 – 17.00.
