Job details:  The Occupational Therapist Assistant will be a key member of the Adult Social Care Community Occupational Therapy Team – duty functions (triage/home visits), providing health and social care services to the local community. The role involves assessing, recommending and providing equipment and minor adaptations and coordinating care for individuals with various needs and working collaboratively with other professionals.
Key Responsibilities:
• Experience Required: Minimum of 1 year in adult social care.
• Assessment: Conduct assessments for minor equipment and adaptations (e.g., grab rails) and complete duty functions as required.
• Care Coordination: Manage a caseload of individuals with diverse and complex needs.
• Collaboration: Work with other professionals to ensure comprehensive provision.
• Data Management: Accurately input information for monitoring and planning purposes.
• Service Efficiency: Aim to provide an efficient and cost-effective service, utilizing appropriate resources from Social Services, Health, the Council, and community organizations.
