Job details: Disability Service, Care and Support.  JD available on request: To undertake person-centred assessments (as directed by Care Act 2014) utilising Occupational Therapy skills and knowledge to develop and implement a support plan, aimed at improving wellbeing and delivering outcomes for adults and children with disabilities. The post holder will also provide advice on specialist equipment/adaptations and housing needs to improve their health and wellbeing and maximise their independence
