Job Details:  Reablement. Occupational Therapist required with knowledge and skills in ASC but also Reablement so has strong goal setting skills too. Must have both Reablement and Adult Social care experience. An OT is required to:- Work closely with Reablement officers (RO’s) to embed their Trusted Assessor (TA) skills, to sign-off TA level equipment competencies (second cohort of RO’s some of whom have already started the self-directed training) and to deliver more training in house on basic equipment as required. Revisit the competencies of some the RO’s from the first cohort of TA training, who are prescribing fewer items of equipment. Ensure a greater range of Reablement strategies are used; including wider range of equipment (outside of TA level), identify minor and make recommendations for major adaptations; use of bespoke fatigue management and falls advice; greater bespoke IAG; activity analysis; greater problem solving and to support the development of improved clinical reasoning. Pilot joint goal setting visits with 3-4 RO’s and track benefits in terms of leaving Reablement with no or lower POC compared to the average and ability of the service to increase the scope of referrals accepted. To hold a small caseload as needed of complex or DHC cases.
